DIGITAL REGISTRATION FORM

Digital format is new this year to make things easier for parents and leaders! Each student MUST have a parent or guardian fill out the student registration form prior to coming to Oasis.

To check form submissions, go to this page. You should have instructions in your email including the security phrase, if not, reach out to oasisconf@gmail.com!

BEFORE YOU SEND LINK TO PARENTS

In order for us to better track whether or not all students in your group have a form filled out, please have every parent fill out the group leader name and church name the same way!

To do this, simply replace firstName & lastName in the link below with your name before you send the link to parents. Do the same for My Church in the link as well. Note: “%20” indicates a space in the url link, so do not delete the %20! Add a %20 as a space in the link you send out if you need more words.

Example

Using the base link which is copiable at the link at the bottom of this page:
(https://form.jotform.com/232504548626054?youthLeader=FirstName%20LastName&churchName=My%20Church%20Name)

John Doe from First Church of Kansas City wanted to pre-fill the form, the end of the link would look like this:

?youthLeader=John%20Doe&churchName=First%20Church%20KC
